HOW I CAN HELP

From a written plan
to everyday support.

Support coordination helps you navigate choices and get services working together. We agree on priorities and the scope of support around your plan and circumstances.

01

Make sense of your plan

Work through what your plan includes, what matters to you and which questions need clarification before you arrange services.

02

Find and compare providers

Explore options around your goals, language preferences, location and service availability. You choose who you work with.

03

Get supports organised

Clarify service agreements, responsibilities and proposed costs with your chosen providers, so everyone understands the arrangements.

04

Bring your support team together

With your consent, communicate with providers, family and other relevant supports. Follow up agreed actions and identify gaps.

05

Prepare for changing needs

Organise questions and relevant information for plan discussions. Review what is working and what may need to change.

06

Build your confidence

Practise asking questions, comparing options and managing everyday service arrangements, at a pace that suits you.

YOUR QUESTIONS

Know what
to expect.

Read the NDIS guide ↗
What is support coordination?

It is practical help to understand your NDIS plan, connect with suitable services and build your ability to organise your supports. Your goals and decisions guide the work.

Can we communicate in Chinese?

Yes. Eric communicates in English and Chinese. Tell us your preferred language when you get in touch. Family members or other supporters can join discussions with your consent.

Can my NDIS funding cover support coordination?

We need to check your plan, available support coordination funding and funding management arrangements. Fees and the scope of service are discussed before you decide whether to proceed.

Can I change my Support Coordinator?

You can discuss a change. Check your current service agreement and any notice requirements first. With your consent, we can discuss a handover, subject to funding and availability.

Who chooses my service providers?

You do. We can help compare options and explain relevant provider relationships so you can make an informed decision. Support coordination does not require you to choose a particular provider.

How can a professional make a referral?

Contact Eric by phone or email to discuss suitability and availability. Obtain the participant’s consent before sharing their information. We will agree on the appropriate next steps together.

LET’S TALK ABOUT YOUR NEXT STEP

You don’t have to
figure it out alone.
